This disclaimer is intended to inform you of the potential risks and benefits associated with our services, which include but are not limited to Intravenous (IV) Therapy, Vitamin Injections, Botulinum Toxin (Botox/Xeomin) Injections, and Semaglutide and other GLP-1 Injections for medical weight loss.

1. No Doctor-Patient Relationship & Not a Substitute for Medical Advice

The information and services provided by RevIVe IV Therapy and Shot Bar are for wellness and aesthetic purposes and are not intended to diagnose, treat, cure, or prevent any disease. Our services are not a substitute for professional medical advice, diagnosis, or treatment from your primary care physician or other qualified healthcare provider.

Receiving services from RevIVe IV Therapy and Shot Bar does not establish a doctor-patient relationship. You should consult with your primary care physician regarding any medical concerns or conditions. Do not disregard professional medical advice or delay in seeking it because of something you have read on our website or have received from our staff.

2. Services Offered

  • IV Therapy & Vitamin Injections: Our intravenous and intramuscular nutrient therapies are intended to replenish hydration, vitamins, minerals, and amino acids to support overall wellness.

  • Botox® & Xeomin® Injections: These neurotoxin injections are used to temporarily improve the appearance of moderate to severe facial wrinkles.

  • Semaglutide and other GLP-1 Injections: These injections are offered as part of a comprehensive medical weight loss program and are intended to be used in conjunction with a healthy diet and exercise.

3. Patient Responsibility and Health Information

It is your responsibility to provide a complete and accurate medical history to our qualified medical staff prior to receiving any services. This includes, but is not limited to, information about any known allergies, medical conditions, medications (prescription and over-the-counter), supplements, and previous cosmetic or medical procedures. Failure to do so may result in adverse effects or contraindications.

4. Assumption of Risk and Informed Consent

All medical and aesthetic procedures carry inherent risks. By agreeing to receive services from RevIVe IV Therapy and Shot Bar, you acknowledge that you have been informed of and understand the potential risks, benefits, and alternatives to the treatments offered. You will be required to sign a specific informed consent form for each treatment you receive, which will provide more detailed information about potential side effects and complications.

A. IV Therapy & Vitamin Injections: Potential risks include, but are not limited to:

  • Discomfort, bruising, pain, or inflammation at the injection site.

  • Allergic reactions, which may be severe.

  • Vein irritation or phlebitis.

  • Infiltration (fluid leaking into surrounding tissue).

  • Infection.

B. Botox® & Xeomin® Injections: Potential risks include, but are not limited to:

  • Bruising, swelling, redness, or pain at the injection site.

  • Headache or flu-like symptoms.

  • Drooping of the eyelid or eyebrow (ptosis).

  • Asymmetry of facial expression.

  • In rare cases, the toxin may spread to other parts of the body and cause serious side effects such as muscle weakness, vision problems, and trouble breathing, speaking, or swallowing.

C. Semaglutide and other GLP-1 Injections: These medications have potential side effects and contraindications.

  • Common side effects include nausea, vomiting, diarrhea, constipation, and abdominal pain.

  • Serious side effects can occur, including pancreatitis, gallbladder problems, kidney problems, and an increased risk of thyroid C-cell tumors. You should not use these medications if you have a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

  • Compounded Semaglutide: Please be aware that if you are receiving a compounded version of Semaglutide, it is prepared by a compounding pharmacy and is not an FDA-approved drug. Compounded drugs are not reviewed by the FDA for safety, effectiveness, or quality.

5. No Guarantee of Results

While we strive to provide the best possible outcomes, the results of our treatments can vary from person to person. Factors such as an individual’s health, lifestyle, and body’s response to treatment can affect the outcome. RevIVe IV Therapy and Shot Bar makes no guarantees or warranties, expressed or implied, regarding the results of any service.

6. Eligibility for Treatment

Our medical staff reserves the right to determine if you are a suitable candidate for our services. We may decline to provide treatment if it is determined that the treatment is not appropriate for you based on your medical history or current health status. Our services are not available to pregnant or breastfeeding women for certain treatments.

7. Post-Treatment Care

You are responsible for following all post-treatment care instructions provided by our staff. Failure to do so may increase the risk of complications and negatively impact your results. If you experience any concerning symptoms after a treatment, you should contact us immediately and, if necessary, seek medical attention from your primary care physician or the nearest emergency room.
